Bereavement Program Outline
LifeNet Health Donor Family Advocacy
The following outline represents the programs and services provided by LifeNet Health's Donor Family Services (DFS) department. Our efforts are intended to comfort and support. From time-to-time our services may change based on input from donor families to best meet the perceived bereavement needs.

- Commitment to Donor Families at Consent and Beyond
- LifeNet Health Communication to Donor Families
- Initial: Bereavement card, Gift for Life booklet, Gift of Life donor medal, organ/tissue disposition letter
- 2 months post-donation: telephone follow-up
- 3 months post-donation: DFS introduction, Angel Catcher journal gift
- 6 months post-donation: Survey of donation experience, bereavement literature
- 9 months post-donation: Quilt message, Writing to Recipients brochure
- 12 months post-donation: Remembrance card
- 18 months post-donation: Final personalized letter, DFS survey, Mourner's Bill of Rights
- Holiday cards
- Donor Family/Transplant Recipient Communication
- Facilitate letters, cards, telephone conversations and e-mail
- Facilitate meetings, if desired
- Other Services
- Bereavement material lending library
- Donor Family Quarterly newsletter
- Annual "In Celebration and Remembrance" donor remembrance tribute
- Research health status of recipients
- Research disposition of tissue donation
- Sponsor donor families at National Donor Recognition Ceremony
- Bereavement and corporate websites for donor families
- Donor family listserve support group
- Periodic grief support groups
- Donor family peer care giving (Grief Companions)
- Workshops on loss and grieving
